Démence à corps de Lewy

 

Critères diagnostiques selon McKeith et coll.1996
1 - Déclin cognitif d'évolution fluctuante interférant avec la vie sociale et le travail. Les troubles de la mémoire ne sont pas majeurs au début.
L'atteinte porte essentiellement sur l'attention et les activités intellectuelles frontales et visuo-spatiales.
2 - Au moins deux des critères suivants:
a) variations du niveau des performances intellectuelles, portant surtout sur l'attention et l'état d'alerte.
b - Hallucinations visuelles bien détaillées
c - survenue spontanée de discrets symptômes extrapyramidaux
3 - Facteurs en faveur du diagnostic:
a) chutes répétées
b) syncope
c) perte de conscience transitoire
d) sensibilité aux neuroleptiques
e) illusions systématiques
f) autres types d'hallucinations
4- Le diagnostic de DCL est moins probable en présence
- d'accidents vasculaires, sous une forme focalisée et vu par imagerie
- présence d'autres pathologies ou d'autres troubles cérébraux expliquant tout ou partie des signes cliniques
